The Hong Kong Society for Rehabilitation (hereunder as ‘HKSR’) is a pioneer in rehabilitation

District Health Centre (DHC) aims to provide primary healthcare services including health promotion,
assessment and patient empowerment program to enhance the public awareness on disease prevention
and the capacity in the self-management of health through the public-private partnership.
Social Worker (Team Leader) (Eastern District Health Centre)

Key responsibilities:
Assist management team to develop, plan, implement and monitor the services in DHC
Develop and implement patient empowerment programs, psychosocial programs, mutual
support services, community resources hub and providing individual counselling
Network with different community partners to organize and execute the health promotion
services
Set up community and patient volunteers scheme in supporting the services
Provide supervision and coach to social worker team staffs to ensure the provision of quality
services
Requirements:
Degree holder or above in social work, Registered Social Worker
Minimum 5 aggregate years or above work experience in social or medical service, especially
in working with persons with chronic illness
Work experience in Primary Health Care service with management capacity and staff
supervision
Team player and good communication and interpersonal skills
Proficiency in the use of IT technology
Competency in stakeholder engagement
Good command of written and spoken English, Cantonese and Mandarin
Shift duty including 1-2 days night shift until 9:00 p.m. per week is required
Work Location: Eastern District
The successful candidate will normally be appointed on contract terms. Renewal of contract will be
subject to the service/operational needs of the department and the candidate’s performance.
